Job summary

PharMerica in Kent, WA is seeking a full‑time Pharmacist to support a non‑retail, closed‑door pharmacy team serving long‑term care and senior living clients. The role emphasizes clinical oversight, collaboration with physicians and nurses, and ensuring safe, effective medication management.

You will review orders, provide therapy recommendations, maintain patient profiles, and participate in monitoring with the healthcare team in a long‑term care setting.

Qualifications

• Bachelor’s degree in Pharmacy required; Pharm D preferred.
• One year experience in long‑term care desired.
• Current and valid pharmacist license in WA.

Responsibilities

• Evaluate physician medication orders for appropriateness, dosage, interactions and administration route.
• Provide clinical recommendations to physicians and healthcare staff on therapy changes.
• Maintain up‑to‑date patient profiles including medical history, labs and current meds.
• Ensure lab tests for monitoring high‑risk meds are ordered and reviewed.
• Collaborate with healthcare professionals to provide clinical support.
• Document communications and interventions in patient records.
