Job summary

NHS Lothian is recruiting a GPhC-registered pharmacist to support delivery of pharmacist-led polypharmacy reviews across care homes and care transitions in Edinburgh. Candidates should have a Master of Pharmacy and progressed PRFT toward prescribing.

The role offers opportunities to develop clinical leadership, work within GP practices and the HSCP, with dedicated development time, supervision from senior pharmacists, and access to a full range of NHS benefits.

Qualifications

• GPhC-registered pharmacist with a Master of Pharmacy.
• Postgraduate clinical experience in patient-focused care.
• Progressed through PRFT toward ready-to-prescribe status.

Responsibilities

• Support GP practices to increase polypharmacy reviews.
• Lead pharmacist-led reviews in care homes and transitions of care in Edinburgh.
• Contribute to reducing hospital admissions and medicines waste.
• Develop and educate others within the team.
