Job summary

ViziRecruiter, LLC. is seeking a Clinical Pharmacist/Pharmacy Specialist for an Ambulatory Care/Oncology/Specialty program in New York. This position involves comprehensive pharmaceutical care in assigned areas and leadership in residency program oversight.

Requires PharmD, NY license, and 3+ years hospital experience; PGY1 preferred and Board Certification in Pharmacotherapy or equivalent with willingness to obtain within a year. English proficiency is required.

Qualifications

• PharmD required with NY license/registration renewal.
• PGY1 residency in applicable specialties strongly preferred.
• Experience >3 years in hospital pharmacy settings.
• Clinical experience with antimicrobial stewardship, anticoagulation stewardship, and/or opioid stewardship preferred.
• Residency program management experience preferred.

Responsibilities

• Participate in all aspects of the medication use system, including formulary management, clinical surveillance, and clinical policy/protocol/guideline development and implementation.
• Provide comprehensive pharmaceutical care in assigned areas.
• Champion and operationalize quality and regulatory compliance initiatives designed to improve medication use and pharmacy practice.
• Assist in the continuous oversight and growth of the residency program.
• Provide medication and practice-related training and education to pharmacy team, members of the patient care team, and patients.

Responsible for ensuring interdisciplinary pharmaceutical care programs are appropriately integrated throughout the institution.

Requirements

Experience: Greater than 3 years of hospital experience as a clinical pharmacist/clinical pharmacy specialist/clinical coordinator/advanced practice pharmacist preferred. Clinical experience with antimicrobial stewardship, anticoagulation stewardship, and/or opioid stewardship preferred. Residency Program management experience preferred.

Licensure, Registration or Certification: Current New York State pharmacist license/registration renewal required. Pharmacotherapy or other relevant Board Certification preferred or willing to obtain within 1 year of eligibility.
