Job summary

Montefiore St. Luke's Cornwall in Newburgh, NY, seeks an experienced pharmacist to provide comprehensive pharmaceutical care in the Emergency Department with variable shifts.

The role focuses on direct patient care, formulary management, and clinical oversight within a dynamic hospital setting. You will participate in medication-use policy development, antimicrobial/anticoagulation/opioid stewardship, and clinical surveillance while educating the pharmacy team and other patient-care staff.

Qualifications

• PharmD required; PGY1 in applicable specialties strongly preferred.
• Must be able to read, write and communicate in English.
• Greater than 3 years of hospital experience as a clinical pharmacist/clinical pharmacy specialist preferred.
• Residency Program management experience preferred.
• Current New York State pharmacist license/registration renewal required;Board Certification preferred or willing to obtain within 1 year of eligibility.

Responsibilities

• Ensure interdisciplinary pharmaceutical care programs are integrated throughout the institution.
• Participate in all aspects of the medication use system including formulary management and clinical surveillance.
• Provide comprehensive pharmaceutical care in assigned areas.
• Champion quality and regulatory compliance initiatives to improve medication use and pharmacy practice.
• Assist in oversight and growth of the residency program; provide training to pharmacy team and patient care team.

Responsible for ensuring interdisciplinary pharmaceutical care programs are appropriately integrated throughout the institution. Participate in all aspects of the medication use system including formulary management, clinical surveillance, and clinical policy/protocol/guideline development and implementation. Provide comprehensive pharmaceutical care in assigned areas. Champion and operationalize quality and regulatory compliance initiatives designed to improve medication use and pharmacy practice. Assist in the continuous oversight and growth of the residency program. Provide medication and practice related training and education to pharmacy team, members of the patient care team, and patients.

Position Qualifications

Education/Training: PharmD required. PGY1 in applicable specialties strongly preferred. Ability to read, write and communicate in English.

Experience: Greater than 3 years of hospital experience as a clinical pharmacist/clinical pharmacy specialist/clinical coordinator/advanced practice pharmacist preferred. Clinical experience with antimicrobial stewardship, anticoagulation stewardship, and/or opioid stewardship preferred. Residency Program management experience preferred.

Licensure, Registration or Certification: Current New York State pharmacist license/ registration renewal required. Pharmacotherapy or other relevant Board Certification preferred or willing to obtain within 1 year of eligibility
