Job summary

Advocate Health Oshkosh Pharmacy is seeking a full-time pharmacist to support inpatient and outpatient IV therapy/oncology services. The role involves variable 8-hour shifts across 1st, 2nd, and occasional 3rd shifts, with roughly one-third of weekends/holidays.

40 hours weekly, benefits eligible, with a competitive pay range. Responsibilities include ensuring medication safety, reviewing orders, and collaborating with physicians and nurses to optimize patient care.

Qualifications

• Minimum of 1 year hospital pharmacy experience preferred.
• Successful completion of an ASHP-accredited PGY1 residency preferred.
• Wisconsin pharmacy licensure is required or pending.

Responsibilities

• Identify, resolution, and prevention of medication-related issues.
• Provide drug information and recommendations to physicians, nurses and patients.
• Develop competency in clinical practice including drug therapy optimization and interaction screening.
• Facilitate medication-related continuity of care at admission, transfer and discharge.
• Review and interpret medication orders accurately.
• Ensure formulary adherence and cost-effective therapies.
• Orient/train new pharmacy team members and supervise technicians.
