Job summary

Advocate Health Oshkosh—Hospital Pharmacy is seeking a full-time pharmacist to support inpatient care and outpatient IV therapy/oncology. The role involves identifying and solving medication-related issues, providing drug information, and ensuring safe, effective medication use across admission, transfer, and discharge processes.

Candidates should have a minimum of 1 year hospital pharmacy experience, with a preferred ASHP-accredited PGY1 residency.

Schedule Details/Additional Information:
  • Variable 8 hour shifts on 1st, 2nd, and occasional 3rds.

  • Will work approximately every third weekend and holiday.

  • Will support inpatient care and outpatient IV therapy/oncology.

A minimum of 1 year hospital pharmacy experience is preferred.

Successful completion of an ASHP accredited PGY1 residency is preferred.

Pay Range: $54.90 - $82.35

Responsible for the identification, resolution, and prevention of medication related issues. Provides drug information and recommendations for optimization of medication therapy to physicians, nurses and patients

Develops competency in clinical practice including appropriate drug therapy for patients and prevention/resolution of drug related problems. Develops competency to evaluate patients for drug-drug, drug-food, drug-lab, and drug-disease interactions. Individualizes drug therapy regimens based on patient’s size, organ functions, and disease states.

Facilitates medication-related continuity of care at admission, transfer and discharge.

Reviews and accurately interprets medication orders.

Develops competency in dispensing and compounding medications consistent with good pharmaceutical practice including compounding technique, packaging, and labeling. Practices in compliance with department policies & procedures, as well as local, state and federal regulations.

Ensures formulary adherence, recommends cost effective therapies to providers, and minimizes waste in daily practice.

Actively supports procedures and initiatives to prevent medication events. Reports events through appropriate channels. Actively supports department medication safety and quality improvement activities.

Orients/trains new pharmacy team members. May work with interns, residents and students to guide daily activities. May be preceptor-in-training. Provides direction to pharmacy technicians to ensure smooth workflow and service standards.
