Cleveland Clinic Fairview Hospital is seeking a Clinical Pharmacist - Inpatient to provide expert pharmacotherapy and pharmacy services for hospitalized patients. The role features a 7 on/7 off evening schedule and opportunities to cross-train in ED/ICU settings.

The position offers a sign-on bonus up to $10,000 and relocation reimbursement up to $5,000, with Magnet Hospital recognition highlighting nursing excellence and collaborative care.

Qualificações

• PharmD or BS in Pharmacy from an accredited college of pharmacy.
• Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation within 90 days of hire.
• Current Basic Life Support (BLS) certification per AHA/Red Cross.
• Current ACLS and/or PALS certification per AHA/Red Cross (within 1 year for applicable roles).
• Post-graduate year 1 residency, fellowship, or two years of relevant pharmacy experience (preferred).

Responsabilidades

The Clinical Pharmacist – Inpatient provides comprehensive clinical and operational pharmacy services to support safe and effective medication therapy for hospitalized patients. Responsibilities include medication order verification and clarification, drug therapy evaluation and dosing optimization, monitoring for interactions and therapeutic appropriateness, providing pharmacokinetic consultations, supporting sterile product preparation, and participating in interdisciplinary rounds when scheduled in a clinical capacity. This caregiver also provides operational pharmacy coverage as needed while ensuring medication safety and regulatory compliance.

This role has the opportunity to cross-train in other specialties such as ED, critical care, etc.

A caregiver in this position will work a 7 on/7off evening shift schedule.

A caregiver who excels in this role will:

• Provide safe, efficient, patient-centered, evidence-based medication services.
• Verify medication orders and/or dispense medications accurately according to state and federal regulations.
• Document services in a complete,accurateandtimelymanner.
• Ensure all pharmacy operationscomply withnational accreditation standards.
• Maintain professional interactions with patients tofacilitatesuccessful patient experiences.
• Educate and train students, residents, other caregivers, and/or patients.

Minimum qualifications for the ideal future caregiver include:

• PharmD or Bachelor of Science degree in Pharmacy from an accreditedcollege of pharmacy
• Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within90 daysof hire
• CurrentBasic Life Support (BLS)certification through the American Heart Association (AHA)or American Red Crossandcompletion of a Board of Pharmacy approved course inthe administration of immunizations or certificationobtained within90 days(for individualsrequiredto administer immunizations).
• Current Advanced Cardiovascular Life Support (ACLS) and/or PediatricAdvanced Life Support (PALS)certification through American Heart Association (AHA)or American Red Cross.For caregiversrequiredto respond to adult or pediatric cardiopulmonary arrest, this isrequiredwithin one year ofhire

Preferred qualifications for the ideal future caregiver include:

• Board certification
• Completion of a post-graduate year 1 residency (PGY1),fellowshipor 2 years of relevant post-graduate pharmacy experience

Physical Requirements :

• Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time.
• Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs.
• Position often includes reaching, stretching, stooping, climbing steps and sitting.
• Must have normal or corrected vision.
• Must have ability toclearly communicate verbally by phone or in person
