Job summary

Penn Medicine seeks a licensed pharmacist to manage clinical and production pharmacy services for assigned patients. Responsibilities include reviewing orders, supervising technicians, sterile compounding as needed, and maintaining complete documentation in patient charts and our information systems.

Hours are generally Monday through Friday, 8:00 AM to 6:00 PM, with required rotation for weekends and holidays per operational needs.

Qualifications

• Pharmacist license required.
• Bachelor’s or PharmD from ACPE-accredited school.
• 1+ year infusion therapies experience (hospital or home) and patient care experience.

Responsibilities

• Provide clinical decisions for patient-specific pharmaceutical care including assessment of medication needs and review of profiles.
• Evaluate for drug interactions, duplicate therapies, and optimize therapy with documentation in medical records.
• Monitor patient responses and lab results, communicate with physicians/nurses/patients, and ensure complete patient databases and regulatory compliance.

Education

Bachelor of Arts/Science (Required)

Pharmacist license (PA) or eligible

ACPE-accredited PharmD or equivalent
