Job summary

Penn Medicine, a leading academic medical center, seeks a PharmD-licensed pharmacist to oversee clinical and production pharmacy services for assigned patients. You will review orders, ensure drug safety, and support infusion therapies across hospital and home settings.

Responsibilities include patient-specific pharmacotherapy decisions, documentation, and ongoing communication with physicians and nurses. The role requires weekend/on-call rotation and a Pennsylvania license.

Qualifications

• Pharmacist must be licensed and registered in PA.
• Experience with hospital/home infusion therapies required.
• Ability to assess patient pharmaceutical needs and optimize therapy.

Responsibilities

• Provide clinical pharmaceutical care and assess patient medication needs.
• Evaluate orders for dosage, interactions, and administration routes.
• Maintain documentation in medical records and patient charts.
• Ensure safety of patients and staff; participate in QA programs.
• Educate staff on drug therapies and infusion devices.
