Job description

The Clinical Pharmacist will support pharmacy quality initiatives to optimize healthcare outcomes. This role will provide a variety of Medication Therapy Management (MTM) services including proactive medication adherence outreach, patient education, and comprehensive medication reviews. The ideal candidate will have a strong clinical background, excellent communication skills, and a passion for helping patients achieve their healthcare goals.

Key Accountabilities
  1. Conduct Comprehensive Medication Reviews (CMRs) to assess medication efficacy, safety, and adherence.
  2. Proactively engage patients to address barriers to medication adherence.
  3. Identify and resolve medication-related problems, including drug interactions, therapy duplications, and cost-saving opportunities.
  4. Provide patient education on proper medication use, potential side effects, and lifestyle modifications to improve health outcomes.
  5. Collaborate with prescribers and pharmacies to recommend medication adjustments.

Strategic goals are focused on enhancing pharmacy-related STARs initiatives and collaborative development of success metrics to evaluate the performance of each program and initiative. The individual in this role will ensure that such work is done in compliance with applicable state and federal regulations.

Qualifications
  1. Bachelor's degree in Pharmacy required; PharmD preferred
  2. 5+ years of related clinical experience
Certifications/Licensure
  1. Valid, registered pharmacy license (in any state)
Skills and Abilities
  1. Prior experience with Medicare Stars is preferred
  2. Experience working in a matrixed or multi-disciplinary environment is preferred
  3. Excellent communication and patient counseling skills
  4. Organized individual who can simultaneously balance and execute through multiple projects and priorities
  5. Self-starter, possessing willingness and ambition to work through ambiguity
  6. Creative thinker

This position is a hub-based role which requires onsite presence. To be eligible for consideration, candidates must reside within a commuting distance to one of the following office locations: Madison, WI, Minnetonka, MN, or Omaha, NE. Onsite frequency is determined by business need and may be up to 2-3 days a week.

The full salary range for this position is $98,400 - $168,600. Salary placement will depend on education, experience, certifications, scope, and other factors. In addition to base salary, this role may be eligible for incentive compensation. Medica offers a comprehensive benefits package including medical, dental, vision, PTO, holidays, paid volunteer time off, 401K, and more.
