Job summary

Harborview Medical Center's Pharmacy Department in Seattle is seeking a Clinical Staff Pharmacist to support ICU and Emergency Department drug therapy, nutrition, and patient care. You will collaborate with medical teams, review regimens, provide education, and counsel patients to ensure safe, efficient medication use.

A Washington pharmacist license and hospital experience are required; residency substitution is allowed.

Qualifications

• Washington State Pharmacist license is required.
• One year of hospital pharmacy practice experience required; residency can substitute month-for-month for experience.
• Completion of ASHP-accredited residency is preferred.

Responsibilities

• Ensure appropriate drug therapy and nutrition, including indication, dose, interactions, and allergies.
• Evaluate pharmacotherapeutic regimens for effectiveness and adverse events; intervene as needed.
• Document pharmacotherapeutic recommendations in medical records and nursing/physician notes.
• Provide drug information and education to medical teams, nurses, and dietitians.
• Counsel patients and provide education on medications.
• Support efficient medication delivery and scheduling; improve distribution accuracy.

The HARBORVIEW PHARMACY DEPARTMENT has an outstanding opportunity for a CLINICAL STAFF PHARMACIST - ICU/EMERGENCY DEPARTMENT.

WORK SCHEDULE

Strong candidates will have an advanced degree or residency, or 2 years of experience in clinically-oriented hospital pharmacy practice.

PRIMARY JOB RESPONSIBILITIES

• Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions and drug allergies
• Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed
• Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations
• Provide drug information and education to medical teams, nursing and dietician staff
• Provide patient counseling and education
• Facilitate efficient medication delivery and appropriate scheduling of medications
• Develop and maintain procedures to improve the efficiency and accuracy of medication distribution and administration

REQUIRED POSITION QUALIFICATIONS

State of Washington Pharmacist License and one year of experience in hospital pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the required experience.

State of Washington Pharmacist license.
