Job summary

Cambridge Health Alliance is seeking a clinically skilled Clinical Pharmacist to join our ambulatory care team. You will provide direct patient care across adult primary care and specialty sites within a hybrid model (telehealth, in-person visits, home visits for select patients).

Responsibilities include pharmacotherapy management, education, and collaboration with physicians and nurses to optimize therapies and outcomes. MA licensure required with credentialing as applicable.

Qualifications

Make a Meaningful Impact Through Ambulatory Care

Cambridge Health Alliance’s Pharmacotherapy Services Department is seeking a dedicated and clinically skilled Clinical Pharmacist to join our innovative ambulatory care team.

Established in 2006, Pharmacotherapy Services provides direct patient care across CHA’s adult primary care and select medical specialty sites. Our team of clinical pharmacists, specialized pharmacy technicians, and patient liaisons works collaboratively with patients and healthcare professionals to optimize medication therapy, improve outcomes, advance medication safety, and support population health initiatives.

This is an opportunity to practice at the forefront of ambulatory pharmacy, with a hybrid care model that includes telehealth, in-person clinic visits, and home visits for select patients. Clinical Pharmacists are integral members of the healthcare team and are recognized as medication-use experts within their areas of practice.

About the Role

Under the general supervision of the Director of Pharmacotherapy Services and in accordance with organizational and departmental policies and procedures, the Clinical Pharmacist provides comprehensive pharmaceutical care within defined practice areas.

The successful candidate will contribute to high-quality, patient-centered care while serving as a clinical resource for colleagues and supporting education, quality improvement, innovation, and development of pharmacy services.

Key Responsibilities
  • Provide direct patient care and clinical pharmacotherapy services in ambulatory care settings.
  • Monitor medication therapy using established clinical criteria to promote safety, effectiveness, and appropriate medication use.
  • Provide comprehensive medication management, including medication selection, monitoring, dosing, and titration within applicable collaborative practice agreements, protocols, and standing orders.
  • Conduct formal and informal medication-use consultations and provide timely, thorough documentation.
  • Serve as a recognized medication-use expert within assigned specialty areas.
  • Provide individualized medication education and counseling to patients, families, and caregivers.
  • Identify, document, and communicate pharmaceutical care interventions and demonstrate the value of clinical pharmacy services to the healthcare team.
  • Promote medication safety and report identified safety concerns through established risk-management processes.
  • Participate in the development, implementation, and ongoing refinement of clinical pharmacy programs.
  • Evaluate the clinical and financial impact of pharmacy services and initiatives.
  • Conduct medication-use evaluations and implement measurable improvements in medication-use processes.
  • Assist with the development and refinement of clinical protocols, guidelines, and standing orders.
  • Participate in the department’s on-call program and appropriately document and communicate outcomes.
  • Collaborate with physicians, advanced practice providers, nurses, and other healthcare professionals across the health system.
  • Support education and professional development through in-services, continuing education, and clinical training.
  • Precept APPE students and PGY-1 pharmacy residents.
  • Contribute to Pharmacy & Therapeutics (P&T) initiatives and new clinical service development.
Qualifications/Requirements
Education/Training:
  • Graduate of an accredited college/university with B.S. in Pharmacy or Pharm.D. degree is required. Board Certification through Board of Pharmaceutical Specialties (BPS) is required within 2 years of employment or satisfying BPS eligibility requirements, whichever is longer.
  • Completion of an ASHP‑accredited pharmacy residency is desirable.
Licensure:
  • Must have a current license in good standing from the Massachusetts Board of Registration in Pharmacy or be eligible for licensure in MA. In lieu of an active MA license, candidates must attain MA licensure prior to completing 1000 hours of work at the Cambridge Health Alliance (CHA). Failure to attain MA licensure within this time period will result in the discontinuation of employment at CHA.
Work Experience:
  • Minimum experience or equivalent outlined below with demonstrated clinical competency required.

Three (3) years' experience as a pharmacist, or completion of a PGY1 residency

Credentialing:
  • Pharmacists will participate in the credentialing and privileging process (Policy Number A-PHA-0006) as directed by their supervisor to establish membership on the Medical Staff as an Allied Health Professional.
