Job summary

DotHouse Health in Boston is seeking a Clinical Pharmacist to join our integrated primary care team. The Clinical Pharmacist serves as a clinical resource for patients, providers and staff and as a strategic thought partner across the health center.

Working collaboratively with primary care providers, nurses, population health staff, and the on-site pharmacy. The Clinical Pharmacist helps optimize medication therapy, improve chronic disease outcomes, and remove barriers to medication access.

Qualifications

  • Graduate of ACPE accredited college of pharmacy with a Doctor of Pharmacy degree.
  • Licensed pharmacist in the state of Massachusetts.
  • Maintains required continuing education for licensure.
  • Completed ASHP Accredited PGY1 with Emphasis in Ambulatory Care or PGY 1 plus PGY 2 in Ambulatory Care preferred.

Responsibilities

  • Work with primary care providers to manage medications for patients with chronic conditions.
  • Review medication regimens for safety, effectiveness, adherence, drug interactions, and cost, and make recommendations to the care team.
  • Review and interpret patient-generated health data, including CGM and remote blood pressure monitoring, and incorporate findings into medication management recommendations.
  • Under collaborative practice agreements, where appropriate, adjust medications and order laboratory monitoring.
  • Meet with patients to review medications, answer questions, discuss side effects, and teach proper use of inhalers, insulin, injectable medications, glucose monitors, and other medication devices.
  • Support medication access by assisting with complex prior authorizations, formulary restrictions, medication denials, and insurance-related barriers to care.
  • Help patients access affordable medications by identifying lower-cost alternatives and manufacturer assistance programs.
  • Perform medication reconciliation after hospital discharge and other transitions of care.
  • Work closely with the on-site pharmacy and clinical staff to coordinate medication-related care.
  • Participate in quality improvement projects related to chronic disease management, medication safety, and clinical quality measures.
  • Document patient encounters and recommendations in the electronic health record.
  • Precept pharmacy students and other learners as appropriate.

Job description

Dothouse Health is a diverse, multi-disciplinary health care and social service agency in Boston’s Fields Corner Neighborhood. Our state-of-the art facilities provide comprehensive clinical services such as Primary Care, Eye Care, Radiology/Mammography, Behavioral Health and Laboratory services. Dorchester House provides over 120,000 visits a year to Adult, Pediatrics and Family Medicine patients.

DotHouse Health is seeking a Clinical Pharmacist to join our integrated primary care team. The Clinical Pharmacist serves as a clinical resource for patients, providers and staff and as a strategic thought partner across the health center. Working collaboratively with primary care providers, nurses, population health staff, and the on-site pharmacy. The Clinical Pharmacist helps optimize medication therapy, improve chronic disease outcomes, and remove barriers to medication access.

Work Hours: 40 hours per week, full time and in person required.

  • Work with primary care providers to manage medications for patients with diabetes, hypertension, asthma, hyperlipidemia, heart failure, and other chronic conditions.
  • Review medication regimens for safety, effectiveness, adherence, drug interactions, and cost, and make recommendations to the care team.
  • Review and interpret patient-generated health data, including continuous glucose monitoring (CGM) and remote blood pressure monitoring, and incorporate findings into medication management recommendations
  • Under collaborative practice agreements, where appropriate, adjust medications and order laboratory monitoring.
  • Meet with patients to review medications, answer questions, discuss side effects, and teach proper use of inhalers, insulin, injectable medications, glucose monitors, and other medication devices in real-time and following primary care visits. Help manage formulary changes by identifying affected patients, recommending alternative medications, and implementing changes under CDTM where appropriate.
  • Support medication access by assisting with complex prior authorizations, formulary restrictions, medication denials, and insurance-related barriers to care.
  • Help patients access affordable medications by identifying lower-cost alternatives, manufacturer assistance programs, and other financial resources.
  • Perform medication reconciliation after hospital discharge and other transitions of care.
  • Work closely with the on-site pharmacy and clinical staff to coordinate medication-related care.

Requirements
  • Graduate of an ACPE accredited college of pharmacy with a Doctor of Pharmacy degree. Prefer ASHP accredited residency. (BPS) Board Certified in a Pharmacy Specialty
  • Licensed pharmacist in the state of Massachusetts. Adhere to the requirements of the Massachusetts Board of Pharmacy for licensed pharmacists.
  • Maintains required continuing education for licensure.
  • Completed ASHP Accredited PGY1 with Emphasis in Ambulatory Care or PGY 1 plus PGY 2 in Ambulatory Care preferred.
Preferred Skills and Competencies
  • Board Certified preferred (BCACP) certified or equivalent certification in area of expertise or willingness/ability to obtain within 2 years of employment
  • 3-5 years of clinical pharmacy experience
  • Experience in a community health center or Federally Qualified Health Center
  • Experience practicing under a Collaborative Drug Therapy Management (CDTM) agreement
  • Experience using the Epic electronic health record (EHR) and comfort documenting clinical encounters, managing in-basket messages, and working within primary care workflows
  • Experience using Microsoft Excel, Word, PowerPoint
  • Experience precepting pharmacy students or residents
  • Fluency in Spanish, Vietnamese, or Cape Verdean Creole
