Job summary

Qualifications

• Doctoral degree PharmD from an accredited college of Pharmacy required
• Equivalent Pharmacy degree from an accredited college of Pharmacy required
• 1–2 years of direct patient care experience in a hospital setting required
• PGY-1 residency preferred
• RPH - Registered Pharmacist licensure in state of practice required
• Board certification preferred

Responsibilities

• Evaluates medication orders for accuracy and resolves discrepancies with physicians or nursing staff
• Assesses feasibility of compounding extemporaneous products and recommends alternatives
• Prepares IV admixtures and sterile products including hazardous agents
• Responds to drug information requests
• Monitors drug therapy and minimizes adverse reactions under supervision
• Reviews patient profiles for contraindications and appropriate drug choices
• Dispenses controlled substances and maintains inventory per regulations
• Supervises technicians and ensures accurate preparation and packaging of medications
• Enters and verifies drug orders and counsels patients when appropriate
• Keeps up-to-date with current pharmaceutical trends and participates in education programs
• Participates in hospital committees and quality improvement teams
