Job summary

Advocate Trinity Hospital – Pharmacy is seeking a pharmacist to oversee pharmaceutical care and provide patient education in a part-time capacity with some weekend availability in the Chicago area. The role emphasizes evidence-based regimen design, safe medication handling, and collaboration with the healthcare team.

The candidate will review prescriptions, participate in emergencies, and mentor pharmacy teammates while ensuring age-appropriate patient care and regulatory compliance.

Qualifications

• Bachelor's or Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy required, or an FPGEC certificate from NABP
• Licensed in the state(s) of practice or eligible for reciprocity required
• Immunization and non-vaccine injectable certification required based on area of practice
• Credentialing and privileging required based on area of practice
• No experience required.

Responsibilities

• Oversees pharmaceutical care and evaluates literature to design therapy regimens.
• Provides counseling on medication therapy, adverse effects, adherence, and administration to patients and caregivers.
• Designs, monitors, and evaluates patient-specific pharmacotherapeutic regimens based on biomedical literature.
• Reviews and interprets medication orders and prescriptions.
• Participates in management of medical emergencies when applicable.
• Supports department medication safety and quality improvement activities.
