Job summary

Mercy Hospital St. Louis is seeking a Clinical Pharmacist to join our hospital team. You will collaborate with physicians, nurses, and other healthcare professionals to ensure safe and effective medication therapy and serve as a drug information resource for staff and patients.

Engage in clinical initiatives, contribute to education efforts, and help deliver high-quality pharmaceutical care in alignment with Mercy standards and values.

Qualifications

• Requires a PharmD or BS in Pharmacy from an accredited school.
• Current pharmacist license in the applicable state.
• Two+ years of clinical pharmacy experience preferred; PGY-1 residency preferred.

Responsibilities

• Review patient records to ensure appropriate medication therapy.
• Assess patient conditions to ensure treatment plans address clinical needs.
• Develop medication plans to minimize risk and optimize outcomes.
• Consult with prescribers, nurses, and patients regarding medications and dosing.
• Provide guidance on proper drug administration and monitor outcomes.
• Serve as a clinical resource for care teams and maintain subspecialty expertise.
• Participate in Medication Therapy Services and dosing/monitoring recommendations.
