Job summary

Mercy Hospital St. Louis is seeking a Clinical Pharmacist to collaborate with physicians, nurses, and the care team to ensure safe and effective medication therapy. The role includes serving as a drug information resource and supporting education and quality initiatives across the medical center.

You will utilize Epic, Micromedex, and automation systems to verify orders, monitor therapies, and contribute to patient outcomes. A PharmD or BS with active state licensure is required.

Qualifications

• PharmD or BS in Pharmacy from an accredited school.
• Current pharmacist license in the applicable state.
• Two+ years of clinical pharmacy experience preferred.

Responsibilities

• Review patient records to ensure appropriate medication therapy.
• Assess patient conditions to ensure treatment plans address clinical needs.
• Develop medication plans to minimize risk and optimize outcomes.
• Consult with prescribers, nurses, and patients regarding medications and dosing.
• Provide guidance on proper drug administration and monitor outcomes.
• Precept and mentor pharmacy students and residents as needed.
• Present in-services and educational sessions for staff.
