Job summary

Option Care Health is seeking a Clinical Pharmacist to provide safe, compliant pharmacy services in the home infusion setting. This entry-level role coordinates patient services with Nursing, educates patients and caregivers, and ensures continuity of pharmaceutical care across settings.

The position requires a PharmD/Pharmacy degree, active license, and knowledge of sterile IV admixture, MTM, or residency experience.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of the following: at least one year of experience in sterile IV admixture procedures or completion of an ASHP residency or equivalent.

Responsibilities

• Determines suitability of individual patients for home care.
• Collects and organizes patient information to determine suitability for home care.
• Assesses patient suitability for home care per Option Care policies.
• Provides medication-use education to Option Care staff, patients and caregivers.
