Job summary

UW Health Care Direct is seeking a pharmacist to provide clinical and distributive infusion pharmacy services for home infusion therapies. The role requires WI licensure and PharmD, with residency or acute care/home infusion experience preferred.

The position emphasizes patient care, medication consultation, accurate documentation in the EHR, and on-call responsibilities in a safety-focused environment.

Qualifications

• Wisconsin state licensure as a pharmacist.
• PharmD degree required.
• PGY-1 hospital residency completed or 2 years acute care experience; 1 year home infusion experience preferred.
• CPR certification for vaccinators; board certification in nutrition support within first year.

Responsibilities

• Coordinate and facilitate infusion care for new and ongoing patients through prescribers, Care Direct team and operations.
• Provide the pharmacist’s patient care process to ensure safe and effective drug therapy.
• Review new patient prescriptions and maintain records per laws and regulations.
• Assess medications, provide recommendations for plan changes, and monitor labs and clinical data.
• Dispense medications and supplies with adherence to federal/state laws and practice standards.
• Document patient notes in the EHR and participate in ongoing education and quality/safety initiatives.
