Job description

Summary:
Reports to the Director of Home Infusion Pharmacy. Responsible for interpreting, evaluating, preparing, and dispensing medications according to legitimate orders, adhering to policies, procedures, and standards of practice for Brown University Health Home Infusion Services. Collaborates with the multidisciplinary team to monitor, assess, and intervene to ensure safe, appropriate, and cost-effective medication use. Reviews and verifies orders for oral and parenteral therapies, including chemotherapy, for appropriateness, drug dosage, route, completeness, potential incompatibilities, and interactions. Clarifies and documents any necessary changes with prescribers. Performs detailed in-process medication checks, sterile compounding, and maintains compliance with regulatory standards, including USP <797> and <800>.

Responsibilities:

  1. Principal Duties and Responsibilities:
  • Collaborate with and serve as a liaison for the interdisciplinary team managing care for Home Infusion patients.
  • Adhere to the policies of the Home Infusion Patient Management Program (HIPMP) and promote patient participation.
  • Participate in developing and evaluating patient care plans.
  • Provide education and consultation to patients and caregivers regarding prescribed medications.
  • Ensure patient-specific parameters are included in medication profiles.
  • Maintain knowledge of current compounding principles, stability, and compatibility standards.
  • Ensure compliance with regulations related to sterile pharmaceutical compounding.
  • Handle hazardous drugs safely, including managing extravasation incidents.
  • Review orders for therapeutic appropriateness and verify medication details.
  • Perform computer activities related to compounding and document accurately.
  • Set up and operate compounding equipment properly.
  • Contribute to safety event reporting and risk minimization.
  • Use health literacy skills for effective patient counseling.
  • Oversee daily pharmacy operations, including aseptic techniques and environment maintenance.
  • Mentor and train technicians, residents, and students.
  • Maintain records of drug purchasing, storage, and distribution in compliance with regulations.
  • Utilize pharmacy systems for reporting and inventory management.
  • Engage in quality improvement initiatives.
  • Participate in professional development activities.
  • Perform other duties as assigned.

Other Information:

  • Basic Knowledge: BSc or PharmD from an accredited college. Must hold or obtain licensure as a pharmacist in Rhode Island.
  • Experience: Completion of PGY-1 residency and/or board certification preferred. Clinical experience may substitute formal training.

Work Environment & Physical Requirements: Standing, walking, manual dexterity, and computer use.

Independent Action: Works independently within policies, referring complex issues to supervisors.

Supervisory Responsibility: None.
