Qualifikationen

• PharmD degree required.
• Current active NJ pharmacist license.
• 2+ years hospital pharmacy experience preferred.
• PGY1/PGY2 residency or 2 years clinical pharmacy experience in the assigned area.

Aufgaben

• Review medication orders and provide recommendations to optimize therapy.
• Conduct medication reconciliation during transitions of care.
• Educate patients and caregivers on proper medication use, side effects, and adherence strategies.
• Collaborate with healthcare providers to develop and implement patient-specific care plans.
• Monitor patient outcomes and adjust therapy to achieve therapeutic goals.
• Precept pharmacy residents and students as needed.
• Document interventions and recommendations in the electronic health record.
• Participate in quality and patient safety initiatives related to pharmacotherapy.

Ausbildung

PharmD degree

NJ Pharmacist license

Hospital pharmacy experience (2+ years) preferred
