Job summary

Responsibilities include monitoring outcomes, adjusting therapy for efficacy and safety, providing drug information, and mentoring pharmacy residents and students.

Responsibilities

• Review patient medication regimens and provide recommendations to optimize therapy based on guidelines.
• Conduct medication reconciliation to ensure accuracy during transitions of care.
• Educate patients and caregivers on proper medication use, side effects, and adherence strategies.
• Collaborate with healthcare providers to develop and implement patient-specific care plans.
• Monitor patient outcomes and adjust therapy to achieve therapeutic goals.
• Participate in interdisciplinary rounds and provide pharmacotherapy recommendations.
• Provide drug information to healthcare professionals and patients as needed.
• Contribute to quality improvement initiatives related to medication use and patient safety.
• Document interventions and clinical assessments in the electronic health record.
• Stay current with developments in pharmacotherapy and evidence-based medicine.
• Serve as a preceptor and mentor for pharmacists, residents and students.
