This Pharmacy position works under the direct supervision of the Director and Clinical Pharmacy Coordinator to assist in optimizing patients’ drug regimens and maintaining proper medication records.

Responsibilities

• Maintain appropriate records for medications and other pharmaceutical supplies ordered by physicians, dentists, and other qualified prescribers.
• Perform dispensing and other duties as assigned.
• Demonstrate knowledge and skills necessary to provide care appropriate to the age of patients served.
• Perform drug regimen reviews to evaluate and assess patients’ medications for related problems, including: identifying therapeutic duplications; reviewing drugs, allergies, drug–drug, drug–nutrient, and drug–laboratory test interactions; evaluating if doses are appropriate for age, renal and liver functions; assisting with patient/family education when necessary; monitoring patients on clinical drug protocols; attending code situations; attending rounds and patient‑care conferences as needed; assisting with pain management as needed; providing drug in‑services to nursing, pharmacy, medical staff, and other ancillary personnel.
• Identify the presence of, or potential for, adverse drug events and report as necessary.
• Perform other duties as assigned.

Qualifications

• Education: BA/BS in Pharmacy or MS/PharmD preferred.
• Licensure: Current California Pharmacy License and current BLS/Healthcare Provider status per the American Heart Association required.
• Experience: Experience in a hospital or other healthcare setting preferred.
• Essential technical/motor skills: Ability to aseptically prepare, mix, and transfer drugs without spillage, especially chemotherapy; proficiency in data entry.
