Job summary

JobRx, Inc. in California is seeking a full-time Clinical Pharmacist to provide clinical pharmaceutical care within our Disease Management Clinic. Responsibilities include medication reconciliation, patient consultations on diabetes and other conditions, and collaboration with healthcare professionals.

The ideal candidate will hold a Doctorate in Pharmacy with experience in hospital pharmacy preferred. A diverse benefits package, including potential sign-on bonuses, is included.

Qualifications

• Doctorate in Pharmacy required.
• Hospital Pharmacy Residency or comparable pharmacy experience preferred.
• Ability to follow and adhere to FDA Health Regulations and other legal compliances.
• Excellent written and verbal communication skills.
• Ability to communicate effectively with patient and medical team members.
• Ability to handle and prioritize tasks in order to meet all deadlines.
• Ability to responsibly handle confidential matters.
• Ability to work in a multi-task, high volume environment.

Responsibilities

• Provide clinical pharmaceutical care to patients.
• Perform Disease Management Programs duties.
• Medication reconciliation.
• Consult patients and physicians on medication issues.
• Monitor patients for side effects and maintain communication.
• Provide concise drug information responses.

Skills

Excellent written and verbal communication skills

Education

Doctorate in Pharmacy
