Job summary

Qualifications

• Pharm.D. or equivalent doctoral degree and active pharmacist license.
• 3 years of graduate education leading to a Ph.D. level as applicable for GS-11.
• Specialized experience in comprehensive medication therapy management and direct patient care.

Responsibilities

• Monitor, modify, and evaluate drug therapy for optimal outcomes; educate and counsel patients.
• Perform sterile and non-sterile compounding, including extemporaneous formulations.
• Evaluate medication orders for appropriateness based on guidelines and patient factors; ensure safety and formulary compliance.
• Dispense medications with attention to accuracy, labeling, and regulatory compliance.
