Job summary

Abrazo Health in Phoenix seeks a Clinical Pharmacy Specialist to develop, implement, and evaluate clinical pharmacy services at Abrazo Central Campus, contributing to education of staff and integration with patient care teams.

Under the direction of the Director of Pharmacy, you will participate in critical care rounds, optimize drug therapy, and help implement drug and disease management protocols, while mentoring students and collaborating with multidisciplinary teams.

Qualifications

• Pharmacist license in the State of Arizona (AZ).
• Pharmacy practice residency or equivalent clinical experience.
• Doctor of Pharmacy degree preferred for advancement.

Responsibilities

• Participates in critical care rounds, optimizing drug therapy.
• Develops evidence-based practices and conducts medication use evaluations.
• Provides education to medical, nursing, and pharmacy staff on clinical initiatives.
• Assists with Pharmacary and Therapeutics Committee meetings organization.
• Other related tasks as assigned.
