Job Description

The Clinical Pharmacist reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards.

The Clinical Pharmacist is responsible for identifying, preventing, and resolving medication-related problems; providing authoritative information to healthcare professionals, patients, and the public; and ensuring that pharmacy services function as an integrated system to provide patient-centered care.

The department aims to deliver the safest, highest quality, most effective, and efficient patient care while developing staff, advancing research, and integrating pharmacy services across Northwestern Medicine.

Responsibilities:

  • Preferred certifications/experience: Critical Care Pharmacy Certification (BCCP), Pharmacotherapy Specialty Certification (BCPS), or 1 year ICU experience
  • Prepare for and attend ICU rounds
  • Precept pharmacy residents during Critical Care rotations
  • Rotate staffing in decentralized ICU positions with other ICU pharmacists
  • Participate in committees, workgroups, and provide education to staff and providers
  • Interpret medication orders for safety and proper use, coordinate problem-solving among pharmacy, nursing, and prescribers
  • Monitor medication therapy for interactions and intervene as needed
  • Evaluate medication appropriateness and communicate with prescribers
  • Act as a pharmacokinetics and adverse reactions consultant
  • Process medication orders into the Electronic Health Record accurately
  • Maintain comprehensive pharmacy patient profiles
  • Oversee medication packaging, labeling, and distribution
  • Compound and dispense pharmaceuticals, including sterile and chemotherapy products
  • Verify medication orders, resolve discrepancies, and review lab data
  • Develop and deliver educational programs for staff and students
  • Educate patients and families about medications
  • Respond to emergency medication management situations
  • Maintain hospital formulary and document clinical activities
  • Communicate effectively with patients and professionals
  • Participate in quality improvement activities
  • Organize workload, oversee activities, and ensure objectives are met
  • Review automated dispensing cabinets and ensure compliance with waste procedures
  • Maintain professional development and expertise
  • Other duties as assigned

Minimum Requirements:

  • Current pharmacy license in Illinois
  • Bachelor’s or Doctor of Pharmacy Degree

Preferred Qualifications:

  • Training or knowledge in neonatology, oncology, or critical care
  • Completion of a residency or equivalent hospital experience
