The Cardiology Care Team pharmacist is responsible for providing comprehensive clinical pharmacy services for patients in cardiac intensive care or step-down units, including order verification, rounds, heart failure consults, discharge counseling, and pharmacokinetic and therapeutic consultation.
The pharmacist is responsible for various aspects of the medication use system, including therapeutic recommendations, advocating for patient safety, preparation, distribution, monitoring, oversight of administration, and billing. They will primarily serve on the Cardiology Care Team but may support other areas as needed, focusing on quality of care through comprehensive medication management.
