Qualifications

• PharmD or BS Pharmacy degree from an AACP-accredited institution.
• Licensed or eligible to become licensed in Florida.

Responsibilities

• Establish relationships with clinic staff and physicians.
• Lead medication management and own clinical outcome KPIs.
• Conduct clinical follow-up with patients.
• Provide 24/7 patient support and guidance.
