Job summary

ProMedica is seeking a Clinical Pharmacist IV at Toledo Hospital to provide comprehensive inpatient pharmacy services, including drug distribution, IV admixture, medical therapy management, drug information, and compliance with controlled substances.

The role includes direct supervision of pharmacy support personnel and adherence to hospital policies, supporting patient care across the medical center and contributing to medication safety and outcomes.

Qualifications

• Pharmacist licensure in good standing in MI and OH.
• 3 years of inpatient pharmacy experience required.
• BLS, ACLS and/or PALS required.
• Board Certification within 2 years of hire preferred.

Job Summary:

As the Clinical Pharmacist IV, you provide comprehensive inpatient pharmacy services, including drug distribution, IV admixture preparation, medical therapy management, drug information dissemination, and compliance with controlled substance regulations.

In this role, you directly supervise pharmacy support personnel.

REQUIREMENTS

• Pharm D degree or Bachelor's degree in Pharmacy with post-2006 experience
• Registered Pharmacist, licensed in good standing in Michigan and Ohio. Single facility-based pharmacists must retain state licensure per site location.
• 3 years of inpatient pharmacy experience
• BLS, ACLS and/or PALS
• Board Certification within 2 years of hire

PREFERRED REQUIREMENTS

• Completion of a PGY1 Pharmacy Residency Training Program
• 4 years of inpatient pharmacy experience
