Job summary

Qualifications

• Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy.
• Active pharmacist license in Connecticut.

Responsibilities

• Provide comprehensive medication management under collaborative practice agreements when applicable.
• Lead 340B Drug Pricing Program activities and ensure HRSA compliance.
• Oversee prior authorization processes and assist providers for insurance approvals.
• Educate patients on medication use and cost-saving options.
• Support transitions of care to reduce readmissions.
