Job summary

The pharmacist will review orders, provide MTM services, and collaborate across care teams, with opportunities for antimicrobial stewardship and quality improvement initiatives.

Qualifications

• PharmD required.
• Active pharmacist license or ability to obtain licensure.
• Residency training and/or clinical pharmacy experience preferred.
• Strong knowledge of medication therapy management and pharmacotherapy.

This position offers the opportunity to work in a collaborative clinical environment focused on improving patient outcomes through evidence-based medication management and interdisciplinary care.

Responsibilities

• Review and verify medication orders to ensure safety, accuracy, and appropriate therapeutic outcomes.
• Provide clinical pharmacy services, including drug information, medication therapy recommendations, and dosing consultations.
• Collaborate with physicians, nurses, and care teams to optimize patient care and medication management.
• Participate in antimicrobial stewardship, formulary management, and quality improvement initiatives.
• Monitor patient therapy, review laboratory results, identify potential drug interactions, and recommend treatment adjustments.
• Provide patient counseling and education regarding medication therapies and disease state management.
• Support pharmacy operations through medication verification, sterile and non-sterile compounding, and technician oversight.
• Serve as a clinical resource and participate in staff education, training, and committee activities.

Ambulatory Care Focus (as applicable)

• Partner with primary care providers to support chronic disease management programs.
• Manage patients with diabetes, hypertension, hyperlipidemia, COPD, CHF, asthma, and other chronic conditions.
• Conduct medication reviews, patient education, and therapy monitoring to improve clinical outcomes.
• Participate in anticoagulation management and medication reconciliation activities.

Qualifications

• Doctor of Pharmacy (PharmD) required.
