Qualifications

• Bachelor of Science in Pharmacy required (PharmD preferred).
• Active Pharmacist license in the state of practice.
• Minimum of: 2 years of acute care pharmacy experience, or 1 year of home infusion pharmacy experience.
• Experience with sterile compounding, IV admixtures, and infusion therapy.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Review and verify medication orders for accuracy, safety, and effectiveness.
• Monitor patient therapies, laboratory values, and clinical outcomes while providing dosing recommendations when needed.
• Collaborate with physicians, nurses, and healthcare teams to optimize patient care.
• Dispense infusion medications, supplies, and equipment while ensuring proper compatibility, storage, and dosing.
• Educate patients and caregivers on infusion therapies, medication administration, and treatment expectations.
• Support sterile compounding operations and maintain compliance with USP , state regulations, and quality standards.
• Provide guidance and support to pharmacy technicians and team members.
