Prime Healthcare's Olympia Fields Hospital in Olympia Fields, IL seeks a Pharmacist to join the Clinical Pharmacy - Emergency Department. This full-time, day-shift role focuses on delivering high-quality pharmaceutical care and patient safety across emergency services.

Under supervision of Pharmacy leadership, you will participate in rounds, counsel patients, and support interdisciplinary teams across medicine, ICU, obstetrics, pediatrics, and peri-operative services.

Qualifications

• Current state Registered Pharmacist License.
• BS in Pharmacy required; Doctor of Pharmacy degree preferred.
• Completion of PGY-1 residency or >3 years of qualifying experience required.
• Postgraduate clinical training PGY-2 preferred.
• Board certification in a Board of Pharmacy Specialties clinical specialty preferred.
• BLS Required; ACLS Preferred.
• Experience in hospital pharmacy operations preferred.

Responsibilities

• Coordinate activities of the pharmacy department to provide high quality pharmaceutical care.
• Direct patient care includes rounding, counseling and participation in care committees.
• Observe and evaluate patients’ medication needs and adjust pharmacotherapy with the care team.
• Participate in services across General Medicine/Surgical, ICU, OB/GYN, Pediatrics, Emergency Care, Oncology, peri-operative, geriatrics and outpatient services.

Overview

Department:Clinical Pharmacy - Emergency Department

Responsibilities

Under the supervision of the Director of Pharmacy and clinical coordinator (if applicable), coordinates the activities of the pharmacy department to provide the highest quality of pharmaceutical care to all patients served by the hospital, while ensuring patient safety. Conducts 50% or more of professional time in direct patient care activities including but not limited to interdisciplinary rounding, patient counseling, or participation in patient care committees. Provision of direct patient care encompasses activities such as the direct observation and evaluation of the patient and his/her medication-related needs; the initiation, modification, or discontinuation of patient-specific pharmacotherapy; and the ongoing pharmacotherapeutic monitoring and follow—up of patients in collaboration with other health professionals. The scope of patient care provided may include: General Medicine/Surgical, Intensive Care Services, Obstetrics & Gynecology, Pediatrics, Emergency Care, Oncology, Peri-operative, Geriatrics and Outpatient Services.

Qualifications
