Northshore Home Health Care is seeking a pharmacist to optimize pharmacotherapy and support safe medication use within an integrated healthcare environment.

You will verify and dispense medications, conduct reconciliation, educate patients, and collaborate with physicians and nurses during rounds to guide decisions and ensure compliance with regulatory requirements.

• Strong knowledge of pharmacotherapy, medication safety, and therapeutic drug monitoring.
• Excellent clinical judgment, attention to detail, and patient-focused communication skills.
