Job summary

Northshore Home Health Care is seeking a clinician focused on safe, evidence-based medication management to improve patient outcomes in home health settings.

You will provide comprehensive medication therapy management, collaborate with physicians and nurses, verify and dispense medications accurately, educate patients and staff, and participate in quality improvement and antimicrobial stewardship efforts.
