Job summary

Providence Holy Cross Medical Center in Mission Hills, CA is seeking a Clinical Pharmacist I to deliver high-quality, individualized pharmaceutical care in collaboration with the healthcare team. This per-diem role emphasizes cost-effective therapies and optimized medication outcomes.

The successful candidate will hold a PharmD or PharmD-equivalent, be California-licensed, and meet BLS and Fire & Life Safety requirements.

Qualifications

The Clinical Pharmacist I has the responsibility for the provision of high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life.

Required Qualifications:

• PharmD in Pharmacy Doctorate or
• Bachelor's Degree in Pharmacy.
• Upon hire: California Pharmacist Registered.
• Upon hire: Basic Life Support (BLS) - where applicable.
• California Fire and Life Safety Card - National Organization within 30 days of hire.

Preferred Qualifications:

• 1 year Acute Care Hospital Pharmacy experience or equivalent experience.
• Completion of ASHP-accredited PGY1 residency.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
