The Job

Apply clinical expertise to perform medication reviews, consultations, and care plan development to prevent medication-related problems and improve outcomes. Evaluate data to enhance program effectiveness and participate in other responsibilities as directed by the Director of Clinical Pharmacy Services.

You Will

• Perform medication reviews to assess appropriateness, identify misuse, and recommend therapy adjustments.
• Develop performance metrics to monitor quality improvements.
• Create and review care plans to optimize medication management.
• Serve as a pharmacy liaison providing training, support, and feedback to medical groups and providers.
• Participate in interdisciplinary team meetings and conduct telephonic reviews with members.
• Develop, implement, and monitor clinical programs aimed at improving quality metrics, medication safety, and health equity.
• Stay updated on CMS rules and regulations to ensure compliance.
• Communicate effectively with stakeholders for successful program execution.
• Document and analyze data to evaluate clinical outcomes.
• Support SCAN’s vision and goals, and perform other duties as assigned.

Your Qualifications

• Pharm D degree required
• Active Pharmacist License in California required
• 3-5 years of relevant experience
• Bilingual in English/Spanish strongly preferred (proficiency test required)
• Advanced analytical skills
• Good problem-solving, communication, and interpersonal skills
• Ability to work collaboratively and foster teamwork
• Strong decision-making skills and attention to detail
• Growth mindset, self-starter, innovative, resourceful
