Job summary

Role Description

The Clinical Pharmacist is a full-time, on-site role based in Germantown, TN, responsible for delivering safe, effective, and evidence-based pharmaceutical care to patients. Daily responsibilities include reviewing and verifying medication orders, performing clinical medication assessments, and collaborating with physicians, nurses, and other care team members to optimize drug therapy. The role also involves preparing and dispensing medications, overseeing sterile and non-sterile compounding, monitoring for drug interactions and adverse effects, and providing medication counseling and education to patients and families. The Clinical Pharmacist participates in clinical rounds, supports antimicrobial stewardship and other pharmacy-led initiatives, and contributes to policy development, quality improvement, and regulatory compliance activities. Documentation of interventions, maintaining accurate records, and supporting pharmacy technicians and trainees are integral parts of the role.

Qualifications

• Demonstrated proficiency in Pharmacy Practice and Pharmacy operations within an acute care or clinical setting.
• Strong foundation in Pharmaceutical Care, including medication therapy management and patient-centered clinical decision-making.
• Solid understanding of Medicine as it relates to pharmacotherapy, disease states, and interdisciplinary care.
• Competence in Compounding, including adherence to sterile and non-sterile compounding standards and relevant regulations.
• Doctor of Pharmacy (PharmD) degree from an accredited institution; completion of a PGY1 residency or equivalent clinical experience preferred.
• Current, unrestricted pharmacist license (or eligibility for licensure) in the state of Tennessee.
• Strong communication and collaboration skills, with the ability to work effectively in a multidisciplinary team and provide clear patient and family education.
• Ability to prioritize, maintain attention to detail, and use clinical information systems and electronic health records efficiently.
• Commitment to evidence-based practice, continuous learning, and participation in quality and safety initiatives.
