Job summary

VIVA USA Inc. is seeking a qualified Pharmacist to perform medication therapy management (MTM) services remotely. The role focuses on outbound calls for comprehensive medication reviews and in-depth consultations on statin use per guidelines.

You will transcribe patient data into a third-party MTM system to generate standardized documents mailed to patients. The ideal candidate has at least 4 years of MTM experience, holds a Florida pharmacist license, and possesses strong communication skills

Qualifications

• 4+ years of clinical MTM pharmacist experience.
• Familiar with Part D Star measures (SUPD, SPC, CMR).
• Pharmacist license in Florida.
• Strong pharmaceutical vocabulary and clinical MTM knowledge.

Responsibilities

• Conduct outbound calls (about 100 per day) for MTM.</br>Perform comprehensive medication reviews (CMR).
• Deliver MTM services and medical consultations.
• Accurately enter patient data into a third-party web-based MTM application.

Position Overview

The pharmacist will be responsible for medication therapy management (MTM) services, including outbound calls for comprehensive medication reviews (CMR) and delivering in-depth consultations on statin medication use per AHA guidelines. The pharmacist will transcribe patient-specific prescription information, diagnosis information, and recommendations into a third-party, web-based application. The data to be transcribed will be either hard-copy or available from a clientapplication. The data entered by the pharmacist will be used to create a standardized document that will be mailed to individual patients. The pharmacist is a member of the MTM product team.

Qualifications

Experience Requirements

• At least 4 years of experience as clinical MTM pharmacist. (Management experience preferred)
• Familiar with driving Part D Star measures. Specifically, SUPD, SPC, and CMR.
• Understanding of pharmaceutical and medical vocabulary
• Clinical experience in medication review.
