Job summary

University Hospitals Pain Management is seeking a Clinical Pharmacist to ensure the safe, effective, and fiscally responsible use of medications across the enterprise. You will apply clinical knowledge and judgment to each patient’s disease states and background while supporting pharmacy care plans.

The role emphasizes accurate order verification, dispensing, medication monitoring, and documentation, with strict compliance to PHI safeguards and UH policies.

Responsibilities

• Accurate order verification
• Accurate dispensing
• Minimize order turnaround time
• Medication monitoring and documentation
• Documentation of clinical activities
• Documentation and performance of medication reconciliation
• Inventory control and adherence to Hospital Formulary policies
• Adherence to local and system Policies and Procedures
• Performs other duties as assigned
• Complies with all policies and standards
• For specific duties and responsibilities, refer to documentation provided by the department during orientation
• Must abide by all requirements to safely and securely maintain PHI for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
