Montefiore Nyack Hospital is seeking a per diem Pharmacist for the Pharmacy department in Nyack, NY. The position involves day shifts with 8.5 hours per shift and a base hourly rate of $68+ per hour, subject to shift differentials and experience.

Ideal candidates will have 2–3 years of hospital pharmacy experience or a PGY-1 residency and hold a New York State Pharmacist license. Willingness to pursue Pharmacotherapy or board certification within a year is preferred.

Qualifications

• 2–3 years hospital pharmacy experience and/or PGY-1 residency preferred.
• Pharmacotherapy or other relevant Board Certification preferred or willing to obtain within 1 year of eligibility.
• New York State Pharmacist license required.

Responsabilités

• Provide safe, effective, and compassionate care, including IV and non-sterile preparation.
• Participate in multidisciplinary rounds and consult with prescribers on treatment options.
• Manage therapeutic monitoring: adjust doses, order labs, notify providers as needed.
• Document all activities in a timely manner.
• Engage in quality improvement activities to improve patient care and pharmacy operations.

Qualifications

1.Experience:

2-3 years hospital pharmacy experience and/or PGY-1 residency preferred.

2.Training/Certification: Pharmacotherapy or other relevant Board Certification preferred or willing to obtain within 1 year of eligibility.

3.Licensure: New York State Pharmacist license

1. Demonstrates sound judgement when providing safe, effective, and compassionate care for patients including solving patient-specific medication related problems, IV and non-sterile preparation, and various aspects of the medication use process.

2. Participate in multidisciplinary rounds/conversations and regularly consult with prescribers concerning best treatment options.

3. Takes responsibility for patients who require therapeutic monitoring of medications including ordering appropriate labs, adjusting medication doses, and notifying appropriate healthcare providers as needed.

4. Documents all required activities in a timely manner.

5. Identifies and engages in organizational and department quality improvement activities to improve patient care, medication use process, and pharmacy operation.
