Responsibilities

• Demonstrates sound judgement in safe, effective, and compassionate patient care including IV and non-sterile preparation and various aspects of the medication use process.
• Participate in multidisciplinary rounds and regularly consult with prescribers on treatment options.
• Monitors patients requiring therapeutic monitoring, orders labs, adjusts meds, and notifies providers as needed.
• Documents all required activities in a timely manner.
• Identifies and engages in quality improvement activities to improve patient care, medication use, and pharmacy operation.

Qualifications

• 2-3 years hospital pharmacy experience and/or PGY-1 residency preferred.

Job Specific Competencies

• Demonstrates sound judgement when providing safe, effective, and compassionate care for patients including solving patient-specific medication related problems, IV and non-sterile preparation, and various aspects of the medication use process.
• Participate in multidisciplinary rounds/conversations and regularly consult with prescribers concerning best treatment options.
• Takes responsibility for patients who require therapeutic monitoring of medications including ordering appropriate labs, adjusting medication doses, and notifying appropriate healthcare providers as needed.
• Documents all required activities in a timely manner.
• Identifies and engages in organizational and department quality improvement activities to improve patient care, medication use process, and pharmacy operation.
