Job summary

Trinity Health in Des Moines, IA is seeking a clinical pharmacist to optimize pharmacotherapy, participate in interdisciplinary rounds, and support safe patient care across service areas including med-surg, critical care, and emergency medicine.

The role requires a pharmacy degree, active or eligible Iowa license, and completion of residency or hospital experience with certifications as appropriate. Preceptorship and EMR documentation are integral parts of daily duties.

Qualifications

• Pharmacy degree from ACPE-approved college of pharmacy.
• Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years hospital experience.
• PGY-2 residency or 3 years of experience in the relevant clinical practice area preferred.
• Licensed or eligible for licensure as a pharmacist in Iowa; licensed within 3 months of hire.
• BLS (within 3 months); ACLS and/or PALS within 3 months based on practice area.
• Board Certification (BCPS or other) within 2 years or 10 years of relevant experience.

Responsibilities

• Perform daily medication therapy reviews and consultations to optimize pharmacotherapy.
• Collaborate with interdisciplinary teams to ensure safe patient care and evidence-based practices.
• Assess provider orders for age/weight/comorbidity appropriateness and allergy checks.
• Participate in medical emergencies and formal alerts (trauma, stroke, sepsis, STEMI).
• Provide drug information and serve as preceptor for students and staff; document care in EMR.
• Assist nursing with medication administration per policies and maintain quality/safety standards.
• Adhere to state/federal laws and hospital policies while supporting the pharmacy mission.

Job description

Clinical pharmacists are responsible for optimizing pharmacotherapy regimens, ensuring safe patient care practices, and advocating for evidence-based medicine within interdisciplinary healthcare teams. As decentralized team members they will conduct daily medication therapy reviews, consults, and other clinical services for patients within designatedarea(s) of responsibility or targeted populations served such as med-surg, telemetry, oncology, pediatrics, critical care, or emergency medicine. They will follow established policies, procedures, protocols, standards, and regulatory requirements in executing job responsibilities as well as contribute to the pharmacy’s mission to improve health outcomes in our community. Clinical pharmacists will be assigned area(s) of coverage based on experience, training, and competency.

Appropriately assesses patient data to identify medication-related problems through profile reviews, pharmacyconsultsand interdisciplinary rounding.

Participates in medical emergencies and other formal alerts such as trauma, stroke, sepsis and STEMI as required.

Reviews and interprets providers orders to assure that the drug, dosage form, and dose is appropriate for the patient's age, weight, and comorbidities. Checks for drug incompatibilities andallergies, andtakes appropriate action when necessary.

As necessary, consult with providers, nurses and other healthcare professionals involved in patient care regarding pharmacotherapy optimization with intent to achieve optimal patient outcomes across the continuum of care.

Provides accurate and timely responses to other healthcare professionals and patients regarding drug information questions and facility medication use policies toassuresafe and appropriate use of medications.

Serves as preceptor for pharmacy students/residents and assists with orientation, instruction and training of pharmacy students, interns, pharmacy technicians, residents, pharmacists and other health professionals.

Assists nursing with medication administration (oral, injectable, continuous infusion titration) depending on the work assignments and in accordance with regulatory and hospital policies.

Documents patient care activities including therapy plan, monitoring and follow-up in electronic medical record as per departmental guidelines.

Responsible for the completion of all assigned work during the shift, while maintaining the highest standards of quality and safety through the completion of quality control checks, identifying and reporting out medication errors and near misses, identifying process improvements and ensuring appropriate documentation.

Maintains current awareness and adheres to State and Federal laws, regulations, pharmacy policies, standard operating procedures, formulary and pertinent clinical practice guidelines while providing pharmaceutical care to patients.

General Requirements:

Pharmacy degree (BS or Pharm.D.) from an ACPE approved college of pharmacy.

Successful completion of ASHP-accredited PGY-1 Pharmacy Practice Residencyor 3 years of hospital experiencerequired.

Successful completion of PGY-2 residencyor 3 years of experience in the relevant clinical practice area preferred.

Licensed, or eligible for licensure, pharmacist in Iowa. Must be licensed within 3 months of hire.

BLS certification within 3 months of hire.

ACLS and/or PALS certification within 3 months of hire based on practice area(s).

BPS Board Certification (BCPS or specialty board certification in the relevant clinical practice area) within 2 years of hireor 10 years of experience in the relevant clinical practice area.

Other certifications if requested by direct supervisors/managers.

Proof of completion of Mandatory Reporter abuse training for child and dependent adult abuse within three (3) months of hire.
