BASIC PURPOSE:


The Clinical Manager will oversee the clinical operations of the New Jersey market ensuring the efficient and compliant delivery of homecare services to elderly patients by Caregivers and Home Health Aides. This role involves overseeing field support staff, compliance and maintaining the highest quality of patient care in a home-based setting. The Clinical Manager will ensure all patient care adheres to regulatory requirements and foster a collaborative environment that aligns with the values and goals of the PACE program.



ESSENTIAL FUNCTIONS:



  • Oversee the day-to-day clinical operations and training for homecare services within the PACE program, ensuring patients receive care in accordance with clinical protocols, policies, and PACE program standards.

  • Provide support to internal departments regarding homecare clinicians, including Caregivers, home health aides, Registered Nurses (RNs), Licensed Practical Nurses (LPNs) and other healthcare professionals.

  • Conduct regular in-home visits to assess the quality and effectiveness of care provided to participants, ensuring services align with personalized care plans and equality and compliance expectations of the company.

  • Monitor caregiver performance, provide feedback, and ensure staff adherence to best practices and regulatory standards.

  • Analyze data from home visits to identify potential risks, inform leadership on continuous quality improvement, and recommend corrective actions to safeguard participant health and safety.

  • Monitor and evaluate the quality of care provided, ensuring compliance with all regulatory standards, including those specific to PACE programs, and addressing any gaps in care or compliance.

  • Provide guidance for ongoing training and professional development for clinical staff to ensure they are well-equipped to meet the needs of patients, are familiar with the PACE model, and remain up-to-date with clinical best practices.

  • Work with Senior Leadership Team to implement initiatives to improve patient outcomes, clinical processes, and operational efficiency, using data-driven insights and feedback from staff and patients.

  • Ensure that all services provided are in compliance with state, federal, and local regulations, as well as PACE program requirements. Participate in audits, surveys, and inspections as needed.

  • Oversee the completion of accurate clinical documentation ensuring compliance with PACE and local healthcare regulations.

  • Conduct thorough audits of compliance records on a quarterly basis to ensure that all documentation, care protocols, and service delivery meet internal standards and regulatory requirements.

  • Assist in the state reporting requirements by preparing and submitting the required reports ensure that all reports are accurate, submitted on time, and aligned with state and federal guidelines.

  • Serve as a point of contact for any inquiries or follow-up regarding state audits or compliance reviews.

  • Other duties as assigned.

QUALIFICATIONS:



  • 5 years of nursing experience, with at least 2 years in a management role within homecare, hospice, or a similar healthcare setting; experience with PACE programs or elderly care is highly desirable.

  • 3 years public health agency (i.e.: government agencies, healthcare & community organizations serving underserved communities, universities and research institutions, etc.)

  • Strong knowledge of homecare regulations, patient care protocols, quality improvement initiatives, and healthcare staffing regulations.

  • Ensure operational excellence and client satisfaction.

  • Excellent leadership, organizational, and communication skills, with the ability to manage multiple priorities in a fast-paced environment.

  • Proficiency with clinical documentation systems, healthcare management software, CRM platforms, and Microsoft Office Suite (Excel, Word, PowerPoint).

  • Strong, problem-solving, and decision-making abilities.

  • Ability to travel to patient homes and clinical locations as needed.



Education: Bachelor's Degree in Nursing (BSN) required; Master's degree in Nursing, Healthcare Administration, or a related field is a plus. Active Registered Nurse (RN) license in the state of assignment or a compact state license is required.



Preferred Skills:



  • Understanding of the Programs of All-Inclusive Care for the Elderly (PACE) model, or similar homecare programs, is highly beneficial.

  • Expertise in clinical assessments, care planning, and coordinating multidisciplinary care for elderly populations.

  • Familiarity with state and federal regulations specific to homecare and elder care, such as Medicare, Medicaid, and healthcare compliance standards.

  • Proven track record in developing and implementing quality improvement initiatives to enhance patient outcomes.

  • Ability to analyze clinical data, track key performance indicators, and generate reports to improve operational efficiency and care quality.

  • Strong ability to handle emergencies or unexpected clinical situations, ensuring swift and appropriate responses.

  • Familiarity with homecare management software, electronic health records (EHR), and scheduling systems.

  • Ability to identify training needs, mentor staff, and foster continuous education to enhance the clinical team's skills and knowledge.

  • Experience building strong relationships with local healthcare providers, hospitals, and community organizations to ensure seamless care transitions.
