Qualifications

• Must have solid knowledge of nursing principles, procedures, and standards of practice as determined by regulations.
• Experience in community or home health is preferred.
• A minimum of one year of licensed professional nursing experience is preferred.

Responsibilities

• Performs initial and ongoing assessments for home health or hospice patients.
• Acts as case manager, coordinating patient/family services with the care team.
• Initiates admissions, re-certifications, and adjusts care levels as needed.
• Provides referrals and specialized nursing procedures and symptom management.
• Prepares all clinical documentation per agency policy.
• Communicates changes in patient condition to physician and updates orders as needed.
• Maintains communication with patient and family to coordinate services and episode management.
• Evaluates care outcomes through performance improvement activities and chart reviews.
• Supervises LPNs and paraprofessionals providing home health care or hospice services.
• Participates in care conferences and quarterly in-service programs.
• Plans discharge from service or bereavement follow-up after hospice death.
