Job summary

LiveWell Homecare Agency is seeking a Clinical Nurse to provide direct patient care in a specialized GI/GU Surgical Unit. The role involves assessing, diagnosing, planning, implementing, and evaluating patient care within established nursing standards.

Key responsibilities include delivering patient- and family-centered care, education on illness management and medications, and collaboration with the healthcare team to develop individualized care plans while maintaining safety and regulatory

Qualifications

• RN licensure required.
• Associate's degree in Nursing or higher.
• Bachelor's degree in Nursing preferred with 2+ years of experience.

Responsibilities

• Deliver patient- and family-centered care, including education on illness management and medication administration.
• Participate in patient safety, quality improvement, and regulatory compliance.
• Develop and document individualized care plans with the healthcare team and ensure effective communication across care settings.
• Assess physical, psychosocial, and emotional needs, respecting patient preferences, values and culture.
• Maintain safety as a priority in all nursing activities.

Qualifications & Skills:

• Associate’s degree in Nursing or higher; RN licensure required.
• Preferred: Bachelor’s Degree in Nursing and at least 2 years of experience.
• Strong critical thinking, communication, and patient education skills.
