Responsibilities include assessment, planning, implementation, and evaluation of individualized care plans, promoting patient and family-centered care, and ensuring safety and quality compliance. Utilizes critical thinking to evaluate physical, psychosocial, and emotional needs, respecting patient values and barriers to involvement. Collaborates with interprofessional teams and documents care activities. The position emphasizes safety, professionalism, and continuous development.
