Job description

Position Title: Clinical Nurse Coordinator, CTS Clinic, Bell Hospital

Position Summary / Career Interest

The Ambulatory Clinic RN Care Coordinator is a licensed registered nurse who provides ethical, evidence-based, safe, and compassionate nursing care. The focus is on patient safety and quality of nursing care by identifying patient needs, performing clinic-related care, conducting health education, promoting advocacy, coordinating healthcare services, and evaluating outcomes. The RN Care Coordinator supports physicians by communicating with patients and families, relaying information, and ensuring understanding of the care plan. They also verify all orders from clinic sessions are completed and communicate results to physicians.

Responsibilities and Essential Job Functions
  1. Manage patient flow, triage, and direct patient care.
  2. Act as liaison among front desk staff, nursing staff, physicians, and clinic management.
  3. Develop patient outcomes, utilize resources effectively, and collaborate with providers on patient goals.
  4. Support clinical and educational programs to enhance patient outcomes and quality of life.
  5. Assist patients in managing their diseases.
  6. Coordinate care continuity after hospital admissions, discharges, and ER visits.
  7. Manage high-risk patients, including those with multiple co-morbidities, using registries.
  8. Build relationships with patients, assessing physical, psychosocial, financial, and environmental needs.
  9. Educate patients and staff about health needs, safety, and benefits.
  10. Use electronic tools like EMR for documentation and tracking progress.
  11. Observe and report patient reactions to treatments; educate about procedures, medications, and wellness.
  12. Maintain and review patient records, test results, and follow-up actions.
  13. Handle treatment-related calls, providing medication and treatment instructions.
  14. Ensure patient confidentiality.
  15. Document care in EMR, maintaining compliance with standards.
  16. Perform competencies required for the assigned unit or department.
Required Education and Experience
  • High School diploma
  • Bachelor's Degree in Nursing
  • Minimum of 2 years RN experience
Preferred Education and Experience
  • Previous medical office experience
Required Licensure and Certification
  • Basic Life Support Heartsaver (BLS HS) from AHA or obtained within 14 days
  • Licensed Registered Nurse (LRN) with multi-state licensure through the State Board of Nursing
  • Current active multi-state RN license or active single-state RN license with proof of application for multi-state license, with deadlines specified.
Knowledge Requirements
  • Basic typing and word processing skills
  • Strong analytical, critical thinking, and problem-solving skills
