Job summary

WVUH West Virginia University Hospitals is seeking an experienced registered nurse to coordinate care for specific patient populations. You will act as a liaison among providers, departments, and schedulers to coordinate testing, referrals and testing logistics.

The role requires RN licensure (or multi-state eNLC), BSN preferred, and certification relevant to the specialized care. Two years RN or five years LPN experience is typical. 40 hours weekly, on-site at multiple locations.

Responsibilities

  • Coordinate plan of care and act as liaison between providers, departments, and schedulers.

  • Triaging patients and giving medical advice when applicable.
  • Monitor patient appointments, testing, and labs for the target population.
  • Assist with prior authorizations and patient education.

MINIMUM QUALIFICATIONS
EDUCATION, CERTIFICATION, AND/OR LICENSURE
  • Current Registered Nurse license issued by the state in which services will be provided or current multi‑state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
  • State criminal background check and Federal (if applicable), as required for regulated areas.
  1. Obtain certification in Basic Life Support within 30 days of hire date.
EXPERIENCE
  • Two years’ experience as an RNOR five years’ experience as an LPN required.
PREFERRED QUALIFICATIONS
EDUCATION, CERTIFICATION, AND/OR LICENSURE
  • Bachelor’s Degree innursing(BSN).
  • Certification relevant to the specialized care they will be providing.
CORE DUTIES AND RESPONSIBILITIES

  • Provides hands on assistance according to the department, patient and provider needs, including but not limited to assisting with procedures, performing tests, preparing or administering patient medications, obtaining specimens.
  • Act as first contact for patients, providing triaging and medical advice where applicable.
  • Coordinatethe plan of care and acts as a liaison between providers, departments, other healthcare professionals and schedulers to coordinate testing, scheduling, referrals etc.
  • Monitors and manages patient’s appointments, testing and labs necessary for the patient population.
  • Assists with obtaining prior‑authorizations for the patient population.
  • Prepares educational material for patient use. Educates and counselspatients concerning medical condition, medication or treatment.
  • Providesspecialized nursing assistance in research efforts when applicable.
  • Provides advice, education and consultation services to nursing staff of any unit that care for the same patient population
  • Attends conferences or educational opportunities to ensure up to date knowledge.
